Patient & Parent Feedback Form
The Cerebral Palsy Integrated Pathway (CPIP) is a physiotherapy-led pathway targeted at children and young people with cerebral palsy and associated conditions. Regular physical examination and radiological investigations allow us to enable early detection and treatment of musculoskeletal conditions that may cause complications if not picked up early.
We are a group of healthcare professionals who are investigating your experience of CPIP as a service user, parent or carer. We would be grateful if you could answer the following questions and provide your contact details if you would be interested in having a more in depth conversation regarding CPIP or any improvements you think could be made to the service.
