Thank you for clicking on this link to complete a questionnaire about your experience of having a newly diagnosed video call with the team psychologist following your recent diagnosis of diabetes.
As we are trialling this new way of working, we are really keen to get your feedback regarding how you felt about the video call, for example what you found helpful or unhelpful about it.
Participation in this is completely voluntary and involves completing this online questionnaire consisting of 19 questions. Some questions are multiple choice and some offer a space to answer freely. If there are any questions you do not want to answer you can select the prefer not to say option or leave the box blank. Your responses or decision not to participate will not affect any aspect of your ongoing diabetes care from the team. If any concerns do arise please use the contact details found below.
You completing this questionnaire allows us to better understand how the psychology service can best support you at this time.
All your answers will be kept confidential and we will not know who has written the responses so as to allow you to answer as openly as you can. You are able to stop taking part at any point up until you have submitted your answers. Please do this by exiting the survey on your browser. Once you have submitted your answers you will be unable to remove your data as we will not be able to identify which is yours. All information will be handled in accordance to UK data protection law.
